  In order to get your information added to our parishioner database, we will need to gather information for you and/or your family. Please complete one of the following:
  1. Completing an online form. Click here for family registration, click here for individual registration.
  2. Print off this document and drop off at the parish office at St. Mary - Lake City or mail it to us at PO Box 131, Lake City, IA 51449.
